All clients of VIVO! MedSpa& Wellness Institute are assured of complete confidentiality regarding all aspects of their treatments and services, as per our HIPPA policy. You have the right to obtain a copy at your convenience.

Appointment Reservations

All appointments require a major credit card to hold your appointment.

Please arrive 15 minutes before your scheduled appointment and complete any necessary paperwork required prior to your treatment. Your punctuality is greatly appreciated and will enable us to dedicate the appropriate amount of time to your scheduled treatment. Arriving greater than15 minutes late may result in the rescheduling your appointment.

When you arrive at your appointment, you will be required to provide your driver’s license.

Scheduling & Cancelation/No Show Policy

Due to the popularity of our medical spa services, it is necessary to make an advance appointment reservation for any of our treatments. By scheduling an appointment, you are agreeing to our Cancelation/No-Show Policy.

We have a 24-hour cancelation policy. If you are unable to make your appointment, please call us no later than 24 hours prior to your appointment to avoid a $50 cancellation fee.

If you fail to show up to your appointment, you will be charged a $100 no show fee for your missed service. You may also go to our website and cancel, if you have a user profile set up.

Patients arriving more than 10 minutes late may result in a shortened appointment or a cancelation if there is not enough time to complete the treatment. If your appointment is cancelled due to late arrival you will be charged the appropriate Cancelation Fee.

Guest Etiquette

Our goal is for each guest to enjoy a tranquil, relaxing experience. We kindly ask you to turn off or silence cellular phones or other electronic devices. To maintain a quiet, professional atmosphere, we ask that all guests consider the volume level of their conversations. Due to safety concerns children younger than 18 years of age are not permitted in the treatment area. For the health and safety of our clients and staff, smoking is not permitted in our facility.

Special Needs

Your comfort and well-being are especially important to us. Prior to receiving your treatment, please inform us of any special needs or medical conditions including pregnancy, hypertension, injuries, allergies, disabilities, or illnesses. Talking with your provider will help customize your treatment to ensure your maximum enjoyment and benefit.

                                                Pricing, Gift Cards, Coupons and Gratuities

For your convenience, we accept cash, Visa, Discover, MasterCard and American Express, and CareCredit.

All prices for treatments and products are subject to change as are policies. At the time of scheduling and appointment, a credit card number is required to secure your reservation. Please note, we do require a scan of your driver’s license upon arrival to your appointment.

Gift cards can be used towards any treatment or product. They are not redeemable for cash. Coupons can NOT be combined and can NOT be applied to any specials or sales on treatments or products. They are only valid towards full-priced treatments and products. Other restrictions may apply, so ask for more details.

In appreciation to the spa staff for outstanding service, gratuities may be given at your discretion.

Dermatology vs. MedSpa Visits

At Vivo MedSpa, we want you to have the best possible experience when it comes to your healthcare services. Part of that experience involves making sure you are fully informed about all aspects of your care, including insurance and billing.

However, we are a Spa;therefore,no treatments will be covered by insurance. We do not accept any insurances.

If you have a dermatology issue, an office visit would need to be scheduled separately through

Erikson Family Medicine (208) 542-7060. They would also bill your insurance for that visit.

What is a dermatology visit?                                                                                                                                                 

Dermatology visits are classified either new patient visits or follow-up visits, during which a concern regarding skin, hair, nails, or mucous membranes is addressed. The evaluation may be extremely focused or may require a more extensive evaluation of the entire skin system.

Dermatology visits usually include the following with your co-pay (if a co-pay is required by your insurance plan):                                                                                    • A review of your medical, surgical, family and social history • An in-depth review of the history regarding your dermatologic concerns • A physical examination • Education and counseling • Prescriptions • Ordering of labs (performance of ordered labs is billed separately)

Many times, the treatment of dermatologic conditions is only achieved, or most effectively achieved, through dermatologic procedures, and not by prescription medicines. Sometimes a combination of both procedural and prescription treatments will provide the most effective means of improvement of your condition.

The following are examples of treatments that may be offered during your dermatology visit, but they are considered procedures, and will be billed separately from the office visit (this list is not all-inclusive):  • Liquid nitrogen treatment (cryotherapy, “freezing” or “burning”) of benign, pre-malignant, or malignant lesions • Intralesional injections • Intramuscular injections • Electrodessication and curettage (“scrape and burn”) • Biopsies •Excisions • Removal of any skin lesions • Incision and drainage of abscesses/cysts • Photodynamic therapy (PDT or Blue Light) • Acne surgery • Patch testing • Wart treatments

Refunds/Returns

All treatments, and gift cards, and coupons are non-refundable.

Legal Policies

Children are not permitted in treatment areas and should be under adult supervision at all times. The safety and well-being of children is the responsibility of the client. Laser or aesthetic services is not a place to bring your children.  We have equipment and lights that may damage them. They are not allowed unattended in the lobby.

I acknowledge and agree that I have been provided a copy of the Vivo MedSpa & Wellness Institute Policies and Procedures that describes the spa’s policies and procedures.  I acknowledge and agree that I have reviewed Vivo MedSpa & Wellness Institute Policies and Procedures in its entirety and agree to comply with the policies and procedures. I acknowledge and agree that I have had all my questions answered to my satisfaction.
